2012 Grant Request Instructions

Grant Request Deadlines

            Winter Deadline:  February 10, 2012          Fall Deadline:  September 30, 2012

***Emergency Grant Requests may be submitted at any time. 

Please note at the top of the form: Emergency Grant***

1.     Name of Requestor is Special Needs Professional (ie: teacher, therapist, medical provider, program director, and others), Parent or Guardian.

2.     Business/School Tax ID number is not needed for personal requests made by a parent or guardian.

3.     In the case of Parent or Guardian request, you must include your Social Security number.

4.     After completing contact information, please indicate which method you would prefer us to use should we have any questions about your grant request.

5.     Form must be complete or your grant request will not be considered for funding.  This includes Tax and Shipping costs.  If there are no tax or shipping costs, then enter N/A. (Estimates are okay.)

6.     If including any literature or pictures of the item(s) requested, please consolidate them to one page if possible and no more than two pages.

7.     If this form has been completed on behalf of someone who may not speak English, please include the name and contact information for the person who can answer any questions we may have.

8.     Salinas Circle for Children prefers to receive all Grant Requests via EMAIL.  If this is not possible, then FAX will be accepted, but ensure that only 1 copy is faxed.  (Please refer to the Grant Request form for Email address or Fax number.)
